Recommendations of Jing'an District on Promoting the Construction of Elderly

Care Service Facilities During the “14th Five-Year Plan” Period

 

With a view to thoroughly carrying out the national strategy of actively responding to population aging, meeting the requirements of the Regulations of Shanghai Municipality on Elderly Care Services, Shanghai's “14th Five-Year Plan” for the Development of Civil Affairs Undertakings, Shanghai's “14th Five-Year Plan” for the Development of Elderly Care Services and the Recommendations on Promoting This Municipality's Construction of Elderly Care Service Facilities during the "14th Five-Year Plan" Period to promote the high-quality development of elderly care service facilities, the following Recommendations are hereby promulgated on the construction of elderly care service facilities in this district during the "14th Five-Year Plan" period:

I. General Requirements

During the "14th Five-Year Plan" period, the construction of elderly care service facilities in this district will focus on "quantity increment, energy enhancement, and efficiency improvement", implement the special layout plan, further weave the network, balance the layout, and promote more professional and high-quality institutional elderly care services and more convenient and accessible community elderly care services, and make family care capabilities continuously enhanced; basic elderly care services will be “guaranteed as much as they should be”, the level of facility security continuously improved, functions continuously improved, and multi-level and diversified elderly care services further and fully developed.

II. Main Objectives

By the end of the "14th Five-Year Plan", the elderly care service facilities in this district will focus on achieving the following goals:

1. Elderly care institutions

By adding new quality and increasing the stock, we will promote the standardization and professional construction of elderly care institutions. The number of new beds in elderly care institutions shall reache the target set by the Municipal Peoples Government. Among them, nursing beds account for more than 60% of the total beds in the district, and no less than 500 nursing beds for cognitive impairment are transformed.

2. Community elderly care service facilities

Comprehensively promote the construction of community-embedded elderly care service facilities. The construction area of community-based elderly care service facilities in the district shall not be less than 40 square meters per 1,000 permanent residents. According to the special planning requirements for the layout of elderly care service facilities in Shanghai, the number and level of facility construction shall be gradually increased. Among them, there are 5 new community-based comprehensive service centers for the elderly; 20 new community-based elderly meal service places; the elderly meal supply capacity reaches 13,500 customers per day.

III. Supporting Policies

1. Supporting measures for elderly care institutions and senior care homes

(1) Basic requirements

In accordance with the Regulations of Shanghai Municipality on Elderly Care Services, the Procedures of Shanghai Municipality for the Administration of Registration and Filing of Elderly Care Institutions (for Trial Implementation) and other relevant provisions, private non-enterprise elderly care institutions and senior care homes registered in Jing'an District, for-profit elderly care institutions that provide basic elderly care services with basic beds, and that meet the following conditions:

1. practicing for at least 6 months;

2. filing in accordance with laws and rules;

3. passed the annual inspection by the management department of the association or completed the publicity of the annual report of the enterprise; and

4. the daily monitoring of service quality being not of "poor" grade (except for those that do not require daily monitoring of service quality according to provisions).

(2) Support measures

A. One-time subsidy for institutional construction stage

The one-time subsidy for the institution building stage is not subject to (1)(2)(3)(4) of the "Basic Requirements", including:

① Subsidy for the construction of new elderly care beds

Newly added elderly care institutions and senior care homes should first apply for municipal subsidies (grants) in accordance with the city regulations. On the basis of obtaining municipal subsidies (grants), the district finance will give a certain proportion of matching subsidies according to the situation, which are divided into:

I. For newly added institutions whose construction funds are guaranteed by financial funds, after obtaining municipal subsidies (grants), the district finance will no longer match the subsidies grants).

II. New institutions whose construction funds are guaranteed by non-financial funds

For the newly-added institutions with their own property rights, after obtaining the municipal subsidies (grants), the district finance will give the matching subsidies three times the municipal subsidies (grants)  

For the newly added institutions without their own property rights, after obtaining the municipal subsidies (grants), the district finance will give a matching subsidy 1.5 times the municipal subsidies (grants)

Ⅲ. New institutions whose construction funds are guaranteed by mixed funds

For elderly care institutions and senior care homes jointly funded by financial and non-financial funds, the subsidy ratio shall be determined by the non-financial fund contributors and the relevant departments raising financial funds through consultation, but the subsidy at the district level shall not be 1.5 times higher than the subsidies (grants) at the municipal level.

② Subsidy for newly added cognitive impairment care beds

For the cognitive impairment care beds set up in elderly care institutions (including senior care homes), it is necessary to first apply for municipal subsidies (grants) in accordance with city regulations. On the basis of obtaining municipal subsidies (grants), financial funds will ensure the construction of new cognitive impairment care beds. After obtaining municipal subsidies (grants), the district finance will no longer match subsidies; funds for newly-added cognitive impairment care beds will be allocated in a 1:1 ratio after obtaining municipal subsidies (grants). Newly-added elderly care institutions that set up cognitive impairment care beds at the same time can enjoy superimposed subsidies for new elderly care beds and cognitive impairment care beds.

③Subsidy for the construction of nursing beds

Nursing beds set up by elderly care institutions (including senior care homes ) and meeting the standards set by this Municipality will be given a one-time subsidy of 2,000 yuan per bed, which will be borne by the municipal welfare lottery public welfare fund and the district finance, each 50%.

Except for the content specified in the document, the district-level subsidy policies for each elderly care bed in the above sections ①②③ shall not be superimposed to enjoy

④ One-time start-up subsidy

For elderly care institutions that have completed legal person registration, the district finance will provide a one-time start-up subsidy at the rate of 5,000 yuan per bed according to the number of approved beds.

For senior care homes that have completed legal person registration, the district finance will provide a one-time start-up subsidy at the rate of 6,000 yuan per bed according to the number of approved beds.

⑤ Subsidy for internal medical institutions

For elderly care institutions (excluding senior care homes) that comply with relevant requirements of Implementation Procedures for "Replacement of Subsidy with Rewards" promulgated by Shanghai Municipal Bureau of Civil Affairs and Shanghai Municipal Bureau of Finance, and that have won the award for built-in medical institutions, the district finance shall match according to the ratio of 1:1. The senior care home with a built-in medical institution that has won the award of built-in medical institutions, the district finance will give a one-time subsidy of 200,000 yuan.

B. Subsidy for the operation stage of the institution

①Rent subsidy

For the elderly care institution and senior care home established by leased properties (excluding institutions funded by the government and operated by a commissioned third party), the district finance subsidizes 35% of the total annual rent of the institution. The total annual rental subsidy conversion standard shall not exceed 2 yuan per square meter per day.

②Daily operation and maintenance subsidy

According to the ratio of the elderly with registered permanent residence in the district to the total number of residents on the last natural day of each month in an elderly care institution and senior care home (referred to as "proportion" in this article, proportion = number of registered elderly people in this district ÷ total number of residents), gradient subsidies are given on a monthly basis. The household registration in the district in this article is the case where the household registration address is displayed as Jing'an District or Zhabei District in the household registration booklet or other household registration certification materials.

For the elderly care institution and senior care home whose proportion does not reach 30% in the month, the daily operation and maintenance subsidy for the month is approved at the standard of 100 yuan per person based on the total number of residents. That is: the monthly subsidy amount = 100 × the total number of residents, and the following terms are analogous.

For the elderly care institution and senior care home with a proportion of 30% but not 50%, in the month, the daily operation and maintenance subsidy for the month is approved at the standard of 120 yuan per person based on the total number of residents.

For the elderly care institution and senior care home with a proportion of 50% but not 70% in the month, the daily operation and maintenance subsidy for the month is approved at the standard of 150 yuan per person based on the total number of residents.

For the elderly care institution and senior care home with a proportion of 70% but not 90% in the month, the daily operation and maintenance subsidy for the month is approved at the standard of 250 yuan per person based on the total number of residents.

For the elderly care institution and senior care home with a proportion of 90% in the month, the daily operation and maintenance subsidy for the month is approved at the standard of 350 yuan per person based on the total number of residents.

For the case where the actual number of elderly people admitted by the institution is higher than the number of rated beds, the "proportion" is calculated based on the number of elderly people with registered permanent residence in the district to the actual total number of residents, and the rated number of operating subsidies is calculated based on the number of rated beds.

③ Comprehensive liability insurance funding

Implementation thereof follows the Notice of Shanghai Civil Affairs Bureau on Implementing Accident Liability Insurance for Elderly Care Institutions (SMCAB G [2008] No. 33) and the relevant requirements of Shanghai Civil Affairs Bureau.

④Professional post subsidy

Where the elderly care institution and senior care home introduce full-time technical personnel with relevant certificates in order to improve the service capacity of the institution, sign employment contracts (agreements) of more than 2 years with the technical personnel, and the technical personnel actually work in the institution, the district finance will give the institution a professional post subsidy of 30,000 yuan/person/year according to the number of professional technicians. Professional technicians include but are not limited to: licensed physicians, nurse practitioners, rehabilitation specialists, social workers, dietary nutritionists, psychological consultants, logistics managers, intermediate fire managers, etc.

For those who hold intermediate and above technical titles and special technical certificates, the rated subsidy standard will be increased by 20%.

If the same professional holds two or more technical certificates and holds corresponding positions at the same time, the rated subsidy standard will be increased by 10%.

If the same professional holds two or more technical certificates, and holds intermediate or above technical titles and technical certificates, and holds corresponding positions at the same time, the rated subsidy standard will be increased by 30%.

If the professional has actually worked in the institution for less than one year, the subsidy amount shall be verified on a monthly basis.

Part-time professionals are not covered by this subsidy.

⑤ Nursing post subsidy

According to the situation of nursing staff in elderly care institutions and senor care homes holding elderly nursing professional qualification certificates, the district finance will provide nursing post subsidies according to the number of certified personnel.

The fifth-level (primary) practice skill level will be subsidized at 20% of this Municipalitys minimum monthly wage in the subsidy year.

The fourth-level (intermediate) practice skill level will be subsidized at 40% of this Municipality’s minimum monthly wage in the subsidy year.

The third-level, second-level, and first-level (advanced) practicing skill levels will be subsidized monthly by 60% of this Municipalitys minimum monthly wage in the subsidy year.

Part-time carers are not covered under this subsidy.

⑥Training subsidy

For the expenses incurred by the elderly care institution and senior care home organizing staff to participate in relevant vocational qualification training and the study of "Elderly Service and Management Major (Specialty)" due to the business needs, the district finance shall grant subsidies as much as 50% of the training expenses according to the actual expenditure of the institution. The actual training cost refers to the net amount remaining after the total training cost paid by the institution deducting the subsidies issued by human resources and social security departments.

⑦Social insurance subsidy

The elderly care institution and senior care home that pay social insurance for their employees in accordance with relevant laws and rules will be given monthly social insurance subsidies to the institution. The subsidy standard is 50% of the total amount paid by the unit in the monthly "Social Insurance Premium Payment Notice" of the elderly care institution and senior care home, and the subsidy amount is rounded exactly to the hundreds digit.

⑧ Electricity subsidy

According to the annual electricity consumption of the elderly care institution and senior care home in the previous year, electricity subsidies will be issued to the 6 months with higher electricity expenses in the previous year, and the subsidy standard is 40% of the total electricity expenses of the institution for 6 months.

The elderly care institution and senior care home that separately charge electricity fees to the elderly shall, according to their electricity consumption in the previous year, grant electricity fee subsidies for the 6 months with higher electricity expenses in the previous year. The subsidy standard is 20% of the institution’s total electricity bill for 6 months.

⑨ Subsidy for elderly care institutions with basic insurance

A certain amount of subsidies will be given to elderly care institutions that are included in the basic insurance category in the district, or have some beds included in the basic insurance category by signing contracts, agreements, and memorandums with government departments.

Subsidy amount of the elderly care institutions included in the basic insurance category = (the rated total of rent subsidies, daily operation and maintenance subsidies, professional post subsidies, nursing post subsidies, and electricity subsidies) × 50% × (the number of elderly people registered in the district who meet this Municipalitys basic insurance elderly care institutions bed occupancy requirements ÷ the total number of residents).

Subsidy amount of elderly care institutions that include some beds in the basic insurance category by signing contracts, agreements and memorandums with government departments = (the total rated amount of rent subsidies, daily operation and maintenance subsidies, professional post subsidies, nursing post subsidies, and electricity subsidies) × 50% × (the number of elderly people registered in the district who meet the bed occupancy requirements of this Municipalitys basic insurance elderly care institutions ÷ the number of elderly people in basic insurance beds) × (number of basic insurance beds / rated number of beds).

In addition, each of the above two types of institutions that accepts an elderly person with household registration in the district who meets the bed occupancy requirements of this Municipalitys basic insurance elderly care institutions, from the month of admission to the month (inclusive) of the elderly person’s departure from the institution, will be given an additional subsidy of 200 yuan per month. On the basis of the publicized charging standards, the institution must give the full amount of subsidy of 200 yuan to the above-mentioned elderly registered in the district in the form of fee reduction or exemption (not in the form of cash refund).

2. Supporting measures for community-based elderly care service facilities

(1) Basic requirements

Community-based comprehensive elderly care service centers, elderly day care institutions, community elderly meal service places, "Joyful-age Home" elderly care service stations, and senior sports health home shall be established in the district with filing or information- registered at the district civil affairs bureau.

(2) A one-time subsidy for the construction phase of the new community-based elderly care service facilities

 A. The community-based comprehensive elderly service center. The municipal welfare lottery public welfare fund will, according to the building area, give each community-based comprehensive elderly service center that has a day care center therein a one-time subsidy of 500,000 to 1 million yuan, and the district finance will match it at a ratio of 1:1. The area standard does not include the area of senior care home set up in the community-based comprehensive elderly service center.

B. The elderly day care institution. For elderly day care institutions of different scales, set up independently, the municipal welfare lottery public welfare fund will give each a one-time subsidy of 150,000 to 600,000 yuan, and the district finance will match it at a ratio of 1:1.

C. The community elderly meal service place. According to the feeding capacity, the municipal welfare lottery public welfare fund will give each a one-time subsidy of 100,000 to 500,000 yuan; the one-time subsidy for an elderly meal place is 10,000 yuan, and the district finance will match it at a ratio of 1:1.

D. The "Joyful-age Home" elderly care service station with the functions of micro-day care, meal assistance and other services for the elderly at home will be given a one-time subsidy of 10,000 yuan by the municipal welfare lottery public welfare fund, and the district finance will match it at a ratio of 1:1.

E. The senior sports health home. According to the construction area, a one-time subsidy of 150,000 to 250,000 yuan will be given, and the municipal welfare lottery public welfare fund and the district finance will each bear 50%.

For the projects of community-based comprehensive elderly care service centers, elderly day care institutions, community elderly meal service places, "Joyful-age Home" elderly service care stations and senior sports health home, the district-level subsidy funds shall not exceed 50% of the actual amount of investment of the facilities. For projects whose construction funds are fully guaranteed by the district finance, the subsidy will not be matched after the municipal subsidy. Projects whose construction funds are guaranteed by non-financial funds shall be subsidized according to the terms. For projects whose construction funds are guaranteed by mixed funds, the subsidy ratio shall be determined by the relevant investors through negotiation.

(3) Subsidy for the operation stage of community-based elderly care service facilities

A. The elderly day care institutions

The district civil affairs bureau shall entrust a third party to carry out a comprehensive evaluation of day care institutions for the elderly. According to the evaluation results, the district finance shall give each institution a maximum operating subsidy of 100,000 yuan.

B. The community elderly meal service places

During the service period, for a nutritious meal provided in the community elderly meal service place, the district finance shall provide a subsidy of 2 yuan.

C. The "Joyful-age Home" elderly care service stations

The district civil affairs bureau shall entrust a third party to conduct a comprehensive assessment of the "Joyful-age Home" elderly care service stations. According to the assessment results, the district finance shall give each service station a maximum operating subsidy of 80,000 yuan.

3. Subsidy Process

(1) Subsidy process for elderly care institutions and senior care homes

A. Application process

①One-time subsidies in the stage of institution construction (5 items in total)

Subsidies for the construction phase of institutions mainly include: subsidies for the construction of new old-age beds, subsidies for new cognitive impairment care beds, subsidies for the construction of nursing beds, one-time subsidies for start-up, and subsidies for internal medical institutions.

Elderly care institutions and senior care homes apply for subsidies for the construction of new elderly care beds and one-time start-up subsidies after completing the registration of private non-enterprise.

Elderly care institutions and senior care homes shall apply for subsidies for new cognitive impairment care beds and nursing bed construction subsidies after completing the acceptance of cognitive impairment care beds and nursing beds.

Elderly care institutions and senior care homes shall apply for subsidies for internal medical institutions after obtaining a medical institution practice license.

② Subsidies for the operation stage of the institution (9 items in total)

Subsidies for the operation stage of the institution mainly include: rent subsidy, daily operation and maintenance subsidy, comprehensive liability insurance subsidy, professional post subsidy, nursing post subsidy, training subsidy, social insurance subsidy, electricity subsidy, and subsidy for basic insurance elderly care institutions. Among them:

a. General liability insurance funding

Implementation thereof shall be done in accordance with the Notice of Shanghai Civil Affairs Bureau on Implementing Accident Liability Insurance for Elderly Care Institutions (SMCAB G [2008] No. 33) and the relevant requirements of Shanghai Civil Affairs Bureau.

b. other subsidies

Elderly care institutions and senior care homes shall, in March each year, apply to the district civil affairs bureau for the previous-year (beyond the year) rent subsidy, daily operation and maintenance subsidy, professional post subsidy, nursing post subsidy, training subsidy, social insurance subsidy, electricity fee subsidy, and the basic insurance elderly care institution subsidy, and submit relevant certification materials.

B. Review and approval

The district civil affairs Bureau shall review the various subsidy application materials submitted by the institution, put forward review opinions and determine the rated amount of each subsidy, and then determine the subsidy amount in different grades on the basis of the rated amount according to the annual standardized construction assessment results. The subsidy is disbursed to the applicant institution, which must issue an invoice that complies with the Law of the People's Republic of China on the Administration of Tax Collection. The main basis for review and approval includes, but is not limited to: review opinions issued by a third-party agency entrusted by the district civil affairs bureau, maintenance data of elderly care institutions in the daily management system of Shanghai elderly care institutions, etc.

(2) Subsidy process for community-based elderly care service facilities

A. One-time subsidy for the construction phase of new community-based elderly care service facilities

The newly-added community-based comprehensive elderly care service centers, elderly day care institutions, community elderly meal service places, "Joyful-age Home" elderly care service stations, senior sports and health home after completing construction and the relevant certification procedures shall apply to the district civil affairs bureau for one-time subsidies for the construction phase

B. Subsidies for the operation stage of community-based elderly care service facilities

According to the comprehensive evaluation results carried out by the elderly day care institutions and the "Joyful-age Home” elderly care service stations, the district civil affairs bureau shall directly approve the amount of the operation subsidy and allocate it to the town and township where the facilities are located.

The district civil affairs bureau shall allocate meal subsidy to the town and township where the facilities are located according to the monthly statistics thereof on the amount of meals served at the meal service places for the elderly in the community.

IV. Supporting Measures

1. Clarify who is responsible

Give full play to the role of the leading group for the construction of the district’s social elderly care service system, formulate specific action plans, clarify time points, strengthen work responsibilities, and ensure various tasks and goals are completed on time according to the overall goals of the district’s social elderly care service system construction of “14th Five-Year Plan” and annual government work goals.

2. Comparing with the target, strengthen the implementation

Combined with the performance assessment of the District Peoples Government and various departments, a process management mechanism shall be established to ensure the completion of various goals. The district-level regulatory detailed planning shall be fully connected with the unit planning, and relevant departments such as civil affairs, planning resources, construction management, and housing management shall do a good job in guiding the construction and development of elderly care facilities.

3. Strengthen the supervision of subsidy funds

Strengthen the performance evaluation and supervision of subsidy funds for elderly care service facilities. Industry supervision departments shall formulate detailed assessment management rules and fund supervision requirements for elderly care service facilities in accordance with these Recommendations, combine management measures with support measures, and guide elderly care service facilities to provide services in a standardized manner. .

V. Other Instructions

During the implementation of these Recommendations, in case of changes in the relevant policies of the State and Shanghai Municipality, adjustments shall be made according to the relevant requirements of the superior.

These Recommendations shall be effective as of March 28, 2022, and be valid until December 31, 2025.
